As Registered Massage Therapists, we follow the guidelines set by the Personal Health Information Protection Act (PHIPA) to ensure the privacy of your information.
It is important that we collect your personal information to allow us to create a proper and safe treatment plan for you. You can be reassured that we take proper care to keep that information private.
WHAT IS PERSONAL INFORMATION?
Personal information is used to identify you. This information includes:
Individual Characteristics: name, age, gender, home address and phone number, and health history
Health: health history, and other health services received
Views and Activities: opinions etc
NOTE: Personal Information does not include business address and phone number.
PROTECTING YOUR INFORMATION
At BodyKNEADS, we ensure your personal information is safe by keeping your paper file in a locked filing cabinet. Electronic information is kept secure with password protection on a computer that is kept in a restricted area with anti-theft precautions.
Online Booking: Your personal information is stored in our online booking software. All data is password protected, encrypted using 256-bit encryption, and is stored on a Canadian server.
We do not collect any Personally Identifiable Information when browsing our site. We may, however, collect information such as time of day, browser type and IP address. This information is used to make improvements on our site.
WHO CAN WE SHARE YOUR INFORMATION WITH?
You: You are entitled to see your own records we have about you. If you believe there is an error in our records regarding factual information, you may request it be corrected. This does not include professional opinions we have formed. You are also entitled to a copy of your file (Note: you may be required to pay a fee for photocopying).
Other Health Care Providers: We may share your information with other health care providers (physiotherapist, doctor etc) with your consent.
Insurance Providers/Lawyers: If your information is needed for viewing from a non-health care provider, we will need you to sign a release form to allow us to share it.
RETENTION AND DESTRUCTION OF INFORMATION
As per our regulating college, we must keep your file for ten years. After this time, all information is shredded.
